Job Description:
A company is looking for a Clinical Inbound Personal Care
Coordinator to assist patients in developing personalized care
plans and achieving health goals.
Key Responsibilities:
Engage with patients to discuss their health needs and coordinate
care plans
Document patient information accurately and maintain compliance
with healthcare regulations
Identify barriers to care goals and escalate complex cases to the
appropriate medical team
Qualifications and Education:
Active COMPACT LPN license
Previous experience in care coordination or healthcare call
centers
Proficiency in EHR and experience working with multiple systems
Adherence to attendance policy and metric standards
