Job Description:
A company is looking for a Virtual Nurse Practitioner to join
their high-risk care team, providing care for geriatric and
clinically complex patients.
Key Responsibilities
Manage patients with complex conditions through tele-assist home
visits and virtual interventions
Conduct thorough initial and ongoing follow-up visits focusing on
chronic condition management
Collaborate with multidisciplinary care teams to impact ongoing
patient care and prevent hospitalizations
Required Qualifications
Completed an accredited NP program with national certification in
Family, Geriatrics, or Adult
5+ years of experience as a Nurse Practitioner, with at least 2
years in complex or geriatric care
Bilingual in Spanish and English highly preferred
Currently licensed in Washington and willing to obtain licensure in
Arizona and other states
Experience with value-based care principles and home-based care
preferred
