Job Summary: Full-Time Registered Nurse (32 hours a week) to cover patients in southern Longmont, Boulder, Erie, and Broomfield. Salary $80,000 to $100,000 per year, paid per visit. Opportunities for career growth, work-life balance, and building trusted nurse-patient relationships.

  As a Registered Nurse, you will:
  
  
  • 
    
  • Initiate, develop, implement, and revise the plan of care in collaboration with the physician and other healthcare professionals.
    
  • Directly and/or indirectly supervise care provided by home health aides and licensed practical vocational nurses, provide instruction as appropriate, and assign tasks according to State and federal regulations.
    
  • Provide high-quality clinical services within the scope of practice and within infection control standards, in accordance with the plan of care, and in coordination with other members of the healthcare team.
    
  • Accurately and timely complete the comprehensive assessments (OASIS) including medication reconciliation.
    
  • Communicate relevant information timely and effectively with appropriate agency staff to ensure quality care and service excellence.
    
    
    You will also:
    
    
    • 
      
    • Participate in the orientation of new agency personnel and serve as a preceptor to other staff and students.
      
    • Actively participate in the survey/survey readiness activities, performance improvement plan, and implement processes and best practices to ensure positive patient outcomes.
      
    • Participate in on-call rotation and weekend rotation as needed to meet patient needs.
      
    • Adhere to and participate in the agency's utilization management model.
      
      
      Requirements:
      
      
      • 
        
      • Current RN licensure in the state of practice
        
      • Current CPR certification
        
      • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
